Employee Benefits Plan Guide

Thank you for choosing to be part of the Child Crisis Arizona team! It is both an honor and a privilege to provide you and your families with access to high-quality health benefits that reflect our commitment to your overall well-being.

We are proud to continue offering a comprehensive and competitive benefits package—including Medical, Dental, Vision, Life, and Disability coverage—in appreciation of your dedication and hard work. For 2026, all provider benefits will remain the same, with a small increase to employee costs towards the continuing elevated prices for medical and dental coverage. We will also continue offering an agency contribution for those enrolled in the HSA plan.

Your benefits are an important part of your total compensation, and we take great care to ensure they remain both affordable and competitive within the nonprofit sector. I encourage you to take time to review your options carefully and select the plan that best supports you and your family. Please submit your 2026 Benefits Enrollment selections via Paycom by Sunday, November 9th.

Continued Partnerships for 2026

We are excited to continue our partnership with Meritain Health  with access to the Aetna Choice POS II (Open Access) network for 2026 
 

We will continue to offer two medical plans, PPO $1,000 and HSA $3,400.  The HSA deductible is increasing from $3,300 to $3,400 per IRS mandate for 2026 in order to maintain Health Savings Account eligibility.


Please see Paycom for Open Enrollment details. This site will provide information on Meritain Health member portal as well as the Aetna Choice POS II

(Open Access) network. 
 

We are pleased to announce we will continue to partner with Cigna for our Dental HMO and Vision Plans, Delta Dental Arizona for our Dental PPO Plan, Lincoln Financial Group for our Life Insurance and Disability plans, we will also be staying with Nationwide for our Voluntary Pet Insurance for the January 1st – December 31st 2026 plan year.

Dental 

We offer a DHMO Dental Plan through Cigna with EyeMed as well as a traditional PPO Dental Plan through Delta Dental Arizona.

Vision 

The Vision Plan will continue with Cigna through the EyeMed Network.

​

Life/AD&D Insurance, Short- and Long- Term Disability & EAP

Child Crisis Arizona will continue to pay 100% of the premiums for these plans on your behalf through Lincoln Financial Group.

Voluntary Pet Insurance 

We will keep offering through Nationwide for our benefits package!

​

​

HSA

Child Crisis Arizona contributes $500 to your HSA account

($250 in April and $250 in October). Child Crisis Arizona will also match your contribution up to a maximum of $1,000.
