Who is Eligible for Insurance?

ALL REGULAR FULL-TIME EMPLOYEES WHO WORK 30 OR MORE HOURS PER WEEK

DEPENDENTS
Your dependents become eligible for coverage when you do, provided you enroll for coverage yourself. Eligible dependents include:

• Your spouse
• Your domestic partner
• Your dependent children up to age 26

DEPENDENT SOCIAL SECURITY NUMBERS

Child Crisis Arizona is required by Medicare legislation to collect Social Security Numbers for your enrolled dependents. When enrolling or renewing your coverage, please have this information with you. Please submit Social Security Numbers for newborns once the card is issued.​

ABOUT DOMESTIC PARTNER COVERAGE
​
Child Crisis Arizona offers medical, dental, vision and voluntary life coverage for your same and opposite gender Domestic Partner along with their eligible dependent children. The premium costs for a Domestic Partner will be the same as the cost for covering a spouse.
This year's open enrollment period is
October 30th-November 9th.
You must log into Paycom during this time to select your benefits for 2026.
